Here’s how to win out when they don’t want to pay: &lt;br /&gt;&lt;br /&gt;1) Out-of-network providers. If you purposely use an out-of-network doctor, you’re probably going to have to pay. But if you had no choice, such as when the hospital chooses the anesthesiologist, you may fall under the “I had no say, so I don’t have to pay” rule, in which the insurer most often has to pony up. You may also find that your insurer is using an out-of-date network provider list – you can often find a current list for your network posted on the Internet. Check it out and inform your insurer if your doctor is indeed on the list. &lt;br /&gt;&lt;br /&gt;2) Your insurer says you’re not covered for certain benefits, such as infertility coverage. Some states require coverage for some benefits, but not others. The best way to find out for sure is to check with your state insurance commissioner. &lt;br /&gt;&lt;br /&gt;3) Your company isn’t paying, but is requesting releases to view your medical history. If they don’t make them, try mail ordering drugs, which is often less expensive. Farmers, State Farm County Mutual, Allstate Indemnity and Mid-Century began notifying state regulators about the increases in late December. &lt;br /&gt;&lt;br /&gt;Under the file-and-use system adopted in 2003, insurers can implement rates immediately after filing them with the department, which then can challenge them upon review and demand refunds.  The insurance companies say the rate increases are necessary because of higher anticipated losses.&lt;br /&gt;&lt;br /&gt;State Farm County Mutual Insurance Co.'s 5.5 percent statewide average increase affects 6 percent of State Farm's 3 million auto customers in Texas.  State Farm County Mutual Insurance Co.'s 5.5 percent statewide average increase affects 6 percent of State Farm's 3 million auto customers in Texas.  The increase is being implemented on all new and renewing customers effective Dec. 31.  Customers of State Farm's other much larger unit, State Farm Mutual Automobile Insurance Co., will not see a rate increase.&lt;br /&gt;&lt;br /&gt;Farmers is imposing a 9.9 percent statewide average increase; and Mid-Century, which is a Farmers unit, is raising its rates an average 4.5 percent. Both went into effect Friday for existing and new customers.&lt;br /&gt;&lt;br /&gt;Allstate's statewide average 12.4 percent increase goes into effect March 7 for existing customers of the company's subsidiary Allstate Fire and Casualty Insurance Co., which insures about 420,0000 policyholders and is the only Allstate unit in Texas taking new auto customers.  Allstate Indemnity customers, which insures many of the company's 1.9 million other customers, will see an average 1.8 percent increase. Although we can`t help you with the knotty Gordian problem, the following recommendations could help you figure out some of the more complicated points of auto insurance. &lt;br /&gt;1) Determine appropriate coverage.&lt;br /&gt;Help control the price you pay, just ask American Insurance Association executive Dave Snyder. For example, Snyder notes that half of your auto insurance bill covers liability and "that has to do with how you are going to use the vehicle, such as for commuting to work and your driving record. If you`ve got a clean driving record, you figure to pay less for insurance than you would if you had a speeding ticket on your record. You can control the other half of your premium which covers damage or loss to your vehicle, comprehensive and collision coverage."&lt;br /&gt;&lt;br /&gt;2) Shop around for insurance.&lt;br /&gt;"In most states," Snyder reports, "there are hundreds of insurers competing for business, so it`s possible to save hundreds of dollars by obtaining quotes from different auto insurance providers." Picking up on Snyder`s theme is his AIA colleague, Nicole Mahrt. Mahrt urges you to work with your insurance provider to get more than one quote. "It pays you to shop around, especially if you feel you`ve been paying too much."&lt;br /&gt;&lt;br /&gt;3) Look for insurance discounts.&lt;br /&gt;"Many insurers will give you a discount if you buy two or more types of insurance from them, for example auto and home insurance," confirms John Marchioni, senior vice president of Personal Lines for Selective Insurance, in Branchville, N.J. More cost-saving suggestions from Marchioni: "Ask about discounts for air bags, anti-lock brakes, daytime running lights and anti-theft devices." &lt;br /&gt;&lt;br /&gt;4) Consider taking a higher deductible.&lt;br /&gt;"You could lower your insurance bill by increasing your deductible," Mahrt says. "But just make sure you can pay the higher deductible if you file a claim."&lt;br /&gt;&lt;br /&gt;5) Look into "stacking" coverages if you file an insurance claim.&lt;br /&gt;Insurance trade group officer Daniel Kummer explains that stacking uninsured/underinsured motorist coverages means "you can collect from more than one of your auto insurance policies. Most states prohibit this practice, but there are about 19 states that either allow stacking or don't address the issue either through legislation or litigation," according to Kummer, director of personal insurance for the Property Casualty Insurers Association of America. "Be sure to check your auto insurance contract to see if it's allowed. "Be advised that you`ll likely pay a higher insurance premium if you have stacked coverage. "It could be 10% to 30% more depending on the litigious nature of the state in which you reside," says Kummer. &lt;br /&gt;&lt;br /&gt;6) Check with your insurance provider BEFORE buying a car.&lt;br /&gt;"Your premium is based in part on the car`s sticker price, the cost to repair it, its safety record and the likelihood of theft," answers Selective`s John Marchioni. Remember to avoid shopping by price alone. "You want an agent and a company that answer your questions and handle claims fairly and efficiently," emphasizes Marchioni, senior vice president of Personal Lines for Selective Insurance.&lt;br /&gt;&lt;br /&gt;7) Notify your auto insurance company as soon as you change companies.&lt;br /&gt;"Be sure to cancel your old policy," suggests PCI`s Dan Kummer. "Do it the same day, but don`t cancel your old policy until you`ve lined up a new contract. That`s important because some states like New York will fine you for the number of days you go without insurance." One last thought from Kummer on the subject: "Most auto insurers specify in your contract that you can terminate your policy any time you want by informing your company in writing about the date you wish that coverage be terminated or you can do that over the phone. &lt;br /&gt;&lt;br /&gt;8) Pick the insurance payment option that best fits your budget.&lt;br /&gt;"Generally, most companies will give you the ability to pay over time, but that comes at a price," says Kummer. "Your payment could increase a few dollars each time you pay by installment. Insurers can accept payments monthly, quarterly, or every six months, what ever is most convenient for you. The survey found most HSA/HDHP policies cover recommended preventive benefits on a "first dollar" basis -- without regard to whether the deductible is met. &lt;br /&gt;&lt;br /&gt;Ninety-nine percent of the HSA/HDHP policies purchased in the large-group market and 96 percent of the small-group market provide first-dollar coverage for preventive care. &lt;br /&gt;&lt;br /&gt;One hundred percent of the HSA/HDHP policies offer first-dollar coverage for adult and child immunizations; well-baby and well-child care; mammography; Pap tests; and annual physical exams and screenings. "The last thing you want is the double whammy of having an accident and then finding out you don't have the right insurance on your RV." &lt;br /&gt;&lt;br /&gt;Your RV is more than a car and insurance for your RV should reflect that. Usually, RV insurance will cover more than what your car insurance covers.&lt;br /&gt;&lt;br /&gt;The NW Insurance Council offers the following tips to help you be sure you've properly insured your recreational vehicle: &lt;br /&gt;&lt;br /&gt;Consider insuring your RV on its own policy, rather than adding it to your auto policy. You keep many items in your RV that you wouldn't normally keep in your car such as furnishings, jewelry, laptops, video recorders and clothing. &lt;br /&gt;Coverage is available in the event of loss or damage to certain items outside the RV up to 25 percent of the replacement cost. &lt;br /&gt;If parked at a campsite, you could be liable if someone is injured in the surrounding area. &lt;br /&gt;If your RV is damaged while you're traveling, you'll need a place to stay and a way to get there. A standard auto policy may not cover these losses or RV towing, which could cost you thousands of dollars. &lt;br /&gt;The high value of many RVs and the potential for extensive physical damage from an accident may mean you'll need additional coverage. Evaluate coverage available through your auto insurance company. Hood has said he didn't negotiate the terms of that settlement and shared the judge's concerns about the deal.&lt;br /&gt;&lt;br /&gt;The state is seeking compensatory and punitive damages against the Bloomington, Ill.-based insurer for an alleged breach of contract. Hood's office filed the suit Monday in Hinds County Circuit Court.&lt;br /&gt;&lt;br /&gt;Hood did not rule out reopening a criminal investigation that he ended in January as part of the agreement. Hood's office was investigating allegations that the company had fraudulently denied policyholder claims after Katrina.&lt;br /&gt;&lt;br /&gt;During a news conference Monday, Hood said the new lawsuit should help thousands of Gulf Coast policyholders, many of whom he says are still living in government-issued trailers because of State Farm's refusal to pay claims.&lt;br /&gt;&lt;br /&gt;"They ought to be ashamed of treating their policyholders like this," he said.&lt;br /&gt;&lt;br /&gt;State Farm spokesman Mike Fernandez said Hood's lawsuit suggests he is "more interested in making headlines in an election year than in making headway for the people of Mississippi."&lt;br /&gt;&lt;br /&gt;"You have to wonder," Fernandez said in a written statement, "what would motivate Attorney General Hood to disrupt an agreement that mirrors the one he was 'happy to announce' on Jan. 23 and asked other insurers to emulate as 'a step to recovery' two days later?"&lt;br /&gt;&lt;br /&gt;After the January settlement agreement fell apart, State Farm reached a separate but similar deal with state Insurance Commissioner George Dale. That pact also calls for the company to reopen and possibly pay tens of thousands of claims, but it isn't subject to judicial oversight.&lt;br /&gt;&lt;br /&gt;State Farm and lawyers for policyholders had presented the original agreement for a mass settlement to U.S. District Judge L.T. It is a fair question that is fairly easy to answer. The next time you receive your auto declarations page in the mail, take a look at the various coverage listed. &lt;br /&gt;&lt;br /&gt;You certainly see Collision &amp; Comprehensive. Those make a good portion of your premium too. But when it comes to items on your policy that have much to do with your "aging" vehicle, that is about it. &lt;br /&gt;&lt;br /&gt;It's the other stuff &lt;br /&gt;&lt;br /&gt;Many insured's simply think about a Total Loss when asking why their premiums are not going down as the age of their vehicle goes up. Sure, your insurance company may give you less each year for that vehicle if it is totalled in an accident or stolen and not recovered. &lt;br /&gt;&lt;br /&gt;But what about your vehicle if it is NOT totalled in a accident or what happens if your vehicle IS recovered after a theft? &lt;br /&gt;&lt;br /&gt;What most consumers may not realize is this - the cost to repair that older vehicle is not going down. Are automotive parts getting less expensive just because your vehicle is a couple of years older? Do Labor costs to fix your vehicle get less expensive each year or more expensive? &lt;br /&gt;&lt;br /&gt;Hopefully, we all can agree on the answers to those two questions. &lt;br /&gt;&lt;br /&gt;When you are at-fault &lt;br /&gt;&lt;br /&gt;What about when you are at-fault in an accident? The bodily injury portion of your policy pays for injuries in the "other" vehicle, not yours. And when you hit someone else, your property damage coverage will fix the "other" vehicle. &lt;br /&gt;&lt;br /&gt;What about when an uninsured or underinsured driver hits you? Those two items have nothing to do with your vehicle. &lt;br /&gt;&lt;br /&gt;Then you can look at glass coverage, road service, car rental, and even medical coverage. Those have zero to do with a total loss of your vehicle. &lt;br /&gt;&lt;br /&gt;Just keep in mind that you and your aging vehicle are covered for more than just a total loss. Many claims are "partial" claims, which require repair to fix. That means your policy will pay for the parts and labor, not a cheap proposition anymore (minus deductibles, of course) &lt;br /&gt;&lt;br /&gt;My auto policy protects my assets too? &lt;br /&gt;&lt;br /&gt;Plus, your auto policy is actually protecting your assets &amp; income if you hit someone else. You may not realize it but Bodily Injury and Property Damage help protect you from being personally liable for the damaged YOU cause to others. This means, that, once you and your family have accrued and paid out a set amount for covered medical care, the insurance company will then pay 100 percent of any covered costs for the rest of that calendar year. &lt;br /&gt;&lt;br /&gt;If you have this feature on your policy, the running tallies I spoke of earlier will enable you to accurately keep track. You surely don't want to pay more than you have to! &lt;br /&gt;&lt;br /&gt;Your Health Insurer Won't Cough Up the Money? &lt;br /&gt;&lt;br /&gt;You've probably heard the old saying that you "Can't fight City Hall!" Insurance companies are even bigger than your average city hall. They're giant conglomerates that hold the reins of a great deal of financial, as well as political, powers, but ... guess what? You can fight City Hall, and you can lock horns, if need be, with your insurance company! &lt;br /&gt;&lt;br /&gt;A common problem is the denial of a claim. Take the denial letter to your Benefits Administrator at your place of employment. Explain the situation to them. The administrator might have the answers "right off the bat" as to why your claim was denied. If he/she can't find a plausible reason, they might call the insurance company for you. If not, at least they can offer professional advice on how to proceed. &lt;br /&gt;&lt;br /&gt;2. You might very well end up calling the provider yourself. If you do, ask to speak to the Claims Processing Department. When you reach the appropriate employee, find out why - in plain English - your claim was denied. &lt;br /&gt;&lt;br /&gt;3. If you're not satisfied with the answer you get, then the next step is to ask to speak to their supervisor.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;You're problem could be a simple mistake, like a wrong diagnosis code, for example. &lt;br /&gt;&lt;br /&gt;This happened to me once: a supplemental insurance company argued that a family member did not spend a part of their hospital stay in the Intensive Care Unit because, according to the billing code, they spent their entire hospital stay in a regular room. Wrong! We disputed the claim. It was found that the wrong code was used, and the problem was resolved. &lt;br /&gt;&lt;br /&gt;4. If you're still sure that your claim should have been accepted, and the insurance company still won't budge, don't give up! There's still hope! &lt;br /&gt;&lt;br /&gt;5. Sit down and draft a letter. Take your time and include all of the pertinent details of your problem. Tell why you think your claim should not be denied; give dates, times, and names if it will help you build your case. After you have finished your draft, go over it carefully. Make sure the information is correct. Then, use your draft to write - up your formal complaint letter. In fact, many students graduate with an average of $3,262 in credit card debt - 10 percent of that group owing more than $7,000 in credit card charges. &lt;br /&gt;&lt;br /&gt;Students forget to factor in other life costs, such as health care, 401K deductions, income taxes, car payments, auto insurance, rent, utility bills, student loans, credit card bills and food expenses into their monthly budget. "After you graduate and land your first job, you do not think about having to pay for all of these expenses," stated a graduate from Ohio University. "Unfortunately, reality sets in pretty fast and you realize you do not have the money to make ends meet - it is a hard lesson to learn!" &lt;br /&gt;&lt;br /&gt;College Debt&lt;br /&gt;Why is there so much credit card debt among college students? "Many credit card companies set up kiosks on college campuses offering free pizzas and t-shirts to try and entice students to sign up for a credit card," noted David Roush, CEO of Insurance.com.
